Like one of the commentors on the site said, the E.R. episode when Mark Greene died and they played this was almost too much. One thing that always bothered me about the song was that he jumbled up the "Over the Rainbow" lyrics, so they're all out of order and don't always rhyme or make sense. I wonder why?
